Amlodipine is an oral drug that belongs to the group of calcium channel blockers. This drug acts by dilating the blood vessels and easing the flow of blood through them. The dilation of blood vessels reduces the pressure exerted on their walls. Amlodipine is mainly used for the treatment of hypertension and some forms of coronary artery disease such as chronic stable angina and vasospastic angina. It can be prescribed either on its own or in combination with other antihypertensive or antianginal drugs.

Important Warnings while taking Amlodipine

One of the side effects that amlodipine could trigger in some patients is low blood pressure. This occurs mostly in people who are sensitive to changes in their blood pressure levels or those with certain cardiovascular problems. Dizziness, weakness, and light-headedness are possible if blood pressure levels drop too much. People suffering from severe aortic stenosis need special medical evaluation before amlodipine is administered.

In some cases where patients suffer from severe obstructive coronary artery disease, the aggravation of angina or myocardial infarction may happen once the medication is introduced. People need to see a doctor immediately if they start experiencing any new symptoms such as angina, serious difficulty breathing, fainting, or other cardiovascular problems.

Patients with severe liver problems may need special medical attention when amlodipine is first taken, as this drug is heavily metabolized in the body by the liver. Older patients and those who are medically weak may also be started on smaller doses of amlodipine based on their condition.

The drug must be taken only according to directions provided by a physician. The dose cannot be increased or decreased arbitrarily by patients.

Things to consider before Amlodipine consumption

  • Allergies
  • Low blood pressure
  • Heart disease
  • Severe aortic stenosis
  • Liver disease
  • Kidney disease
  • Swelling or edema
  • Pregnancy and breastfeeding
  • Other blood pressure medicines
  • Other prescription medicines and supplements
  • History of dizziness or fainting
  • Age and overall health

Amlodipine is used to reduce blood pressure and therefore can lead to the development of such adverse effects as dizziness, low blood pressure, and swelling of ankles and/or other parts of the body. Patients suffering from the advanced stage of aortic stenosis or coronary heart disease are advised to discuss their cardiovascular status with a health care provider before the initiation of therapy. In case of liver dysfunction, the processing of the drug will be affected, so the initial dose of the drug will have to be adjusted accordingly. The patient is required to inform their health care professional about any other drugs they take, especially antihypertensive drugs and simvastatin, as amlodipine can affect the level of some of the drugs.

Application/Consumption

This drug is orally ingested once a day and can be consumed either with or without food. This medicine needs to be consumed in the morning at about the same time every day as per the prescription. This medicine must be swallowed in a glass of water and in the prescribed dose only.

Amlodipine is consumed either individually or in conjunction with another medicine that controls hypertension or angina pain. With regular use, the condition of hypertension can gradually get better; therefore, patients need to consume this drug even if they do not suffer from any signs of high blood pressure.

Missed doses should be ignored, and instructions from the healthcare professional should be followed without ingesting an extra dose. Patients should not stop consuming this medicine without proper consultation with a doctor since discontinuation of medication allows symptoms to appear again. Hypertension should be monitored on a regular basis. Patients should also notify their doctor if they experience chronic swelling, dizziness, fainting, and chest pain.

Dosing information

For adults with hypertension, the recommended starting dose of amlodipine is generally 5 mg once daily, with a maximum recommended dose of 10 mg once daily. Small or frail patients, older patients, and patients with hepatic insufficiency may be started at 2.5 mg once daily.

The dosage for angina and coronary artery disease is generally individualized according to the patient's response and medical condition.

Uses

Amlodipine is used for:

  • Hypertension: It lowers high blood pressure and helps reduce the risk of cardiovascular events such as stroke and myocardial infarction.
  • Chronic stable angina: It helps prevent or reduce episodes of chest pain caused by reduced blood flow to the heart.
  • Vasospastic angina: It is used for angina caused by coronary artery spasm.
  • Certain coronary artery disease: It may be used in appropriately selected patients with angiographically documented coronary artery disease.

Amlodipine works mainly by blocking calcium entry into vascular smooth muscle cells. This causes relaxation and widening of the arteries, reducing peripheral vascular resistance and helping lower blood pressure. It also improves blood flow to the heart muscle in patients with certain forms of angina.

Amlodipine side effects

The most frequent side effect of amlodipine is edema, or swelling, especially of the ankle and foot areas. The risk of edema may rise with increased dosages. Other potential side effects of amlodipine include fatigue, nausea, stomach pain, and drowsiness.

Patients may also suffer from dizziness, flushing, headache, palpitations, or a sensation of weakness. This may happen since amlodipine affects blood vessel tone and decreases blood pressure. Patients suffering from persistent or severe dizziness should consult their healthcare provider about this condition.

The swelling of the ankles or feet is usually connected to the dilation of small blood vessels and is not necessarily a sign of fluid retention due to heart failure. Nevertheless, any new or worsening swelling needs to be reported, especially for those patients having cardiovascular disease.

Sometimes amlodipine leads to hypotension, which results in dizziness, weakness, or faintness due to a drop in blood pressure. Worsened angina or myocardial infarction may occur after initiation or up-titration of amlodipine, especially in patients with significant coronary artery obstruction. The onset of new, worsening, or severe chest pain necessitates medical consultation.

Amlodipine can have drug interactions. One such interaction occurs between amlodipine and simvastatin. The current prescribing information indicates that the dose of simvastatin should be limited to 20 mg per day when used with amlodipine since amlodipine can increase the level of simvastatin in the blood.

Another potential interaction concerns the addition of antihypertensive medications to amlodipine due to their additive blood-pressure-lowering effect. The dose of amlodipine should be selected with care in cases of severe hepatic impairment because amlodipine is metabolized extensively in the liver.

Older patients might be more susceptible to the blood pressure-reducing effect of amlodipine; therefore, starting treatment within the lower part of the recommended dose range is advisable. Continuous monitoring will assist in determining the appropriateness of the dose prescribed. If one experiences severe chest pain, fainting, extreme difficulty breathing, or any symptom indicating a severe cardiovascular adverse reaction, the patient must see a doctor immediately.
