Application/Consumption
It is an oral medication that is taken once in a day. The FDA approved Flomax capsules have to be taken around 30 minutes after taking the same meal every day. The constant association with food will ensure that drug exposure remains constant.
It should be taken as a complete pill without breaking, chewing, or opening it. Patients should follow the directions carefully and not increase the dose on their own.
In case of missed doses, it is recommended by the FDA label that patients start with 0.4 mg once in a day dose. As dizziness and hypotension can occur, patients should be careful when rising from a sitting or lying position, especially during the first few days of starting therapy and increasing the dose.
Dosing information
The usual recommended starting dose of tamsulosin for BPH is 0.4 mg once daily, taken approximately 30 minutes after the same meal each day.
For patients who do not respond adequately to 0.4 mg after 2 to 4 weeks, the dose may be increased to 0.8 mg once daily under medical supervision.
The dose should be individualized according to symptom response and tolerability. Patients should not increase or decrease their dose independently.
Uses
Tamsulosin is primarily used for:
- Treatment of signs and symptoms of benign prostatic hyperplasia.
- Improvement of urinary flow associated with prostate-related bladder outlet obstruction.
- Reduction of symptoms such as weak urine stream, difficulty initiating urination, urinary frequency, urgency, and incomplete bladder emptying.
- Tamsulosin works by relaxing smooth muscle in the prostate and bladder neck. It does not treat prostate cancer and is not indicated for hypertension.
Tamsulosin side effects
Common tamsulosin side effects include dizziness, abnormal ejaculation, headaches, nasal congestion, fatigue, back pain, and diarrhea. The reason for abnormal ejaculation or lack of ejaculation may be associated with the effect of the drug on smooth muscles responsible for this function.
It is especially important to pay attention to dizziness and orthostatic symptoms, as they can increase the risk of falling. Patients with serious cases of dizziness, fainting, or significant fatigue should consult a healthcare professional.
Tamsulosin can sometimes lead to hypotension and syncope. The risk increases when it is taken in combination with other drugs that lower blood pressure, including some PDE-5 inhibitors, used to treat erectile dysfunction. Patients should follow their healthcare professional’s instructions regarding these medications.
Tamsulosin is also linked to intraoperative floppy iris syndrome. This condition may arise in patients undergoing cataract surgery even if they have stopped taking tamsulosin. That is why patients should inform their eye surgeon about their taking of this drug. Allergic reactions to tamsulosin are quite rare and may manifest in the form of rash, itching, hives, swelling, or difficulty breathing.
Priapism is an uncommon side effect. An erection that is either too long or too painful needs prompt medical attention to avoid erectile dysfunction. Drug-drug interaction is an issue to consider. Tamsulosin is a drug mainly metabolized by CYP3A4 and CYP2D6, and thus drugs that may block the action of these enzymes increase the levels of tamsulosin. As indicated by the FDA label, tamsulosin interacts significantly with potent inhibitors of CYP3A4 enzymes, such as ketoconazole. Combination with another alpha-blocker is also not recommended.
