Innovaccer Inc., a leading San Francisco-based company known for its analytics and AI skills used to support health systems with collective health initiatives, acquired a tech-enabled platform aiming to transition specialty care by shifting from episodic to consistent digital models. The buying for an open sum highlights the company’s fourth-best acquisition in the past two years, the company proudly mentioned. The acquisition, formed on the last acquisitions involving cured, pharmacy quality solutions, humbi AI is expanding its ‘healthcare intelligence cloud’ to the agentic care augmentation. In the past few years, Innovaccer claimed that it has built a strong foundation of an integrated healthcare operating system at the core, including patient engagement and data unification, actuarial intelligence, and pharmacy quality.
Now, agentic care augmentation is the next feature to be added to this robust foundation. Healthcare innovation has largely involved Story Health’s collaboration with health systems, consisting of ChristianaCare in Delaware and Utah’s Intermountain Health. Last year, in February, the CEO Tom Stanis of Story Health and the MD medical director of the center for heart and vascular health at ChristianaCare, Kirk Garratt, had mentioned their partnership and how they are about to advance health outcomes for hypertension and heart failure patients in Wilmington, Del, in one of the interviews.
The collaboration opts for Story Health’s digital hybrid platform that enables patients to work with an excellent health coach to cross-check that their care plan is followed and the treatment mission is accomplished. With this approach, ChristianaCare stated that it has been able to identify a few health disparities with a prominent improvement in the black patient population, followed by prescribing doses of professionally referred medical therapy for heart failure. A former Google and Verily Life Sciences executive, Stanis, precisely elaborated the three parts regarding what exactly Story Health offers. They create a technology platform and excellent care planning tool that merges best with the electronic health record to support the care plan of the clinicians for the patients, more than just a clinic visit, involving medication management for patients.
Clinicians can fetch the clinical data from the EHR combined with data directly from the patient’s home to keep a check on their health. The second part describes patient experience to support them with the guidance via all these tools and services, and alert them on what should be done. Stanis said, “It can work through text-based communication, including SMS, with the patient so they can operate through basic/common phones.”
